The role of the social worker provides many services to patients and the hospital community: (1) knowledge and practice within the NASW Code of Ethics guidelines, (2) provide bio-psychosocial assessment of individuals, (3) implement intervention and treatment plans that promote client well-being and ensure a continuum of care, (4) case management to facilitate collaboration among providers, (5) discharge planning, and (6) act as educators/counselors for clients, families, community and other professionals regarding disease prevention, impact of illness and disease progression, advocacy for benefits, health maintenance, and adherence to treatment regimens.
(including but not limited to) issues of abuse(adult or child), psychiatric issues, adoptions, unusual maternity issues, financial assistance, DNR, end of life, homeless, and substance abuse
Acute patients such as Intensive Care Unit, Step-down Unit, Medical Units, Surgical Unit and Inpatient Rehabilitation Unit
Also included are outpatient services/patients in departments of: Emergency, Cardiac Rehabilitation, Pain Clinic, Wound Clinic, Ambulatory as well as other ancillary services

Social workers available Monday through Friday from 8:00 a.m. to 4:30 p.m. On-Call social worker may be reached via the operator/on-call care coordinator. A social worker is also available in the Emergency Department Monday through Friday from 10:30 a.m. to 7:30 p.m.
